Viagra is an oral drug used in men with erectile dysfunction. Its pharmaceutical name is sildenafil. Sildenafil was developed in 1992 to treat angina pectoris and coronary heart disease, however, clinical trials have shown its ability to improve erection. Mechanism of action of Generic Viagra

Cyclic guanosine monophosphate (cGMP) is involved in the erection mechanism. It is destroyed by the PDE enzymes in the body. PDE5 is a special form found exclusively in the cavernous body of the penis. PDE5 inhibitors such as sildenafil slow down the breakdown of cGMP, which greatly enhances erection. The effect is achieved only in the presence of sexual stimulation.

Dosage and administration

The initial recommended dose is 50 mg, followed by a possible dose change, either decrease to 25 mg or increase to 100 mg depending on the effect and tolerability. Sildenafil is taken 1 time per day, 1 hour before the intended sexual intercourse. The action of the drug begins 40–60 minutes after administration and lasts for 3–5 hours, and according to some reports, up to 12 hours.

Despite the high efficacy of sildenafil, in some patients, it does not cause any improvement, often due to incorrect administration. At the beginning of treatment, it is recommended to take sildenafil on an empty stomach at least 40 minutes before sexual activity. Importantly, the effect develops only against the background of adequate sexual arousal and largely depends on it. In addition, studies have shown that in some patients the maximum effect of sildenafil is achieved by the 6–8th dose. Side effects of Generic Viagra

Like other PDE5 inhibitors, Viagra has a good safety profile. The most common side effects are headache (7%), flushing of the face (7%), and indigestion (2%). Others include nasal congestion, dizziness, muscle pain, and temporary visual impairment, usually in the form of blue coloring. These side effects in most cases are easily tolerated and do not require treatment. Most patients do not consider them essential and do not refuse further medication. Side effects requiring medical attention are exceptionally rare and may include priapism, low blood pressure, cardiovascular problems, and hearing and vision impairment. The frequency of side effects decreases with further treatment. Studies have shown that at the beginning of treatment, headaches were reported by 7% of the patients, while after 16 weeks the number was reduced to 1%.

Contraindications

Sildenafil is contraindicated in patients taking nitrates, people with hypotension, severe hepatic insufficiency, pigmentary retinopathy, and in case of recent myocardial infarction. The drug is not recommended for use by men who should not lead an active sex life due to severe heart failure.

Sildenafil is contraindicated for simultaneous use with Ritonavir. When used in combination with Ketoconazole, Erythromycin, Cimetidine, and other inhibitors of the CYP3A4 enzyme, it is necessary to reduce the daily dosage of sildenafil to 25 mg.
